Welcome to the Alluvion Health Foundation, a dedicated entity committed to enhancing and expanding the reach of Alluvion Health’s mission to create and inspire healthier lives. As a pivotal component of the Alluvion Health ecosystem, our foundation harnesses the collective strength and talents of our staff, board members, and community supporters to bring transformative healthcare services to North Central Montana. With a steadfast commitment to innovative and comprehensive care, we strive to address and bridge the gaps in healthcare services, ensuring that every individual in our community has access to quality care.

The Alluvion Health Foundation operates as a separate legal entity with its own governance and board of directors, working in close collaboration with Alluvion Health to fulfill shared goals. Our mission is to support Alluvion Health’s unique approach to healthcare, which focuses on whole-person, whole-life care. This encompasses not only medical services but also behavioral health, dental care, and a variety of supportive services aimed at overcoming barriers to care. By engaging with our community and fostering strong partnerships, we endeavor to make a lasting impact, creating a healthier and more vibrant community for all​​.

MOBILE AUTISM clinic

The Mobile Autism Clinic at Alluvion Health will represent a significant step forward in providing accessible and specialized care for children with autism spectrum disorders in North Central Montana. This innovative mobile clinic will be designed to reach families in remote and underserved areas, ensuring that high-quality diagnostic and therapeutic services are available to all, regardless of geographic location. By bringing comprehensive autism care directly to the communities, the Mobile Autism Clinic will address barriers to access and also support early intervention, which is crucial for the best developmental outcomes.

Ways of Giving

Cash Support

This is the simplest form of charitable giving, and any amount is appreciated! Your tax deduction is equal to the amount you have donated with a record of receipt. We will provide proof of return with our pledge form and a prompt thank you for your generous gift of improving our overall community’s health!

Securities/Stock

This is one of the most tax-efficient ways to contribute to your charity over the long term, and there are several advantages to this method of giving. Call Teresa Schreiner today to discuss the benefits of donated giving through stocks. To facilitate a gift of securities the donor (or transferring broker) should contact the foundation directly prior to transfer at (406) 231-6521.

Planned Giving

A wonderful way to support the future of Alluvion Health is to provide for a future gift. There are two types of charitable gifts in a life income gift - a charitable gift annuity or charitable remainder trust, or through a charitable lead trust. Contact the Foundation for questions about charitable trusts at (406) 231-6521.

Bequests

The most common form of deferred giving is a bequest in a person’s will or revocable (“living”) trust. Bequests are a wonderful way to have an impact beyond your lifetime and to leave a meaningful legacy that will make a difference in our community for many years to come.

Life Income Gifts

Life income gifts benefit both the foundation and the donor. Donors may receive tax and financial benefits by creating a life income gift, such as a charitable gift annuity or a charitable remainder trust through which you can receive fixed payments.

Pooled Income Fund

These are creative ways for individuals to generate income while also giving smaller portions to charity. Through a fund, donors can “pool” together different securities and/or combine them with cash to create a larger amount of money to distribute to charity. Money is paid out to the donor and any other beneficiaries that have pooled together assets to the fund.

In-kind Goods and Services

Donated goods, services, time/labor, use of facilities, and equipment are valuable gifts for our Foundation. We record the fair market value of each in-kind good and service as revenue, and, as such, the donor can take a tax deduction for it. Introduction

Teresa Schreiner joined Alluvion Health in April 2021 as a Program Development Director and in October of 2021, she was promoted to the Interim Executive Director of the Foundation Board. In March of 2022, Teresa was appointed to the permanent Executive Director position by the Board of Directors. With experience in community development and fundraising, and a passion for this community, Teresa is eager to bring out the philanthropic and coordinating efforts with Alluvion Health to meet the needs ofour most vulnerable.

Prior to joining Alluvion, Teresa worked in economic development as the region’s Development Director for the Great falls Development Authority. She also worked for the Department of Labor in Great Falls at the Job Service Operations Bureau.

Teresa has served on many nonprofit and public boards and local committees, current commitments include Great Falls Kiwanis, United Way Graduation Matters and Education Initiative Steering Committees , Kid’s Education Yes, and the Continuum of Care. Of her past commitments, Teresa is most proud of her elected service as a school board trustee during 2017 – 2020 for Great Falls Public Schools.

Introduction

Heidi Hockett joined the Alluvion Foundation Board in April 2022 and feels that it is an honor to represent Alluvion Health as they are one of the most important healthcare organizations in Great Falls. Alluvion Health goes beyond clinic visits-they get to the heart of the individual and family needs with wraparound services which are a safety net to those in need. She has been in the energy sector many years and is currently the manager of Community Relations for NorthWestern Energy. She completed her undergraduate studies at Weber State University receiving her Bachelor’s degree in Criminal Justice. She furthered her education with two Master’s degrees, the first from Montana State University in Rehabilitation and Counseling and the second in Business Administration from the University of Mary. Heidi proudly serves on the following boards: Rotary of Great Falls, Big Brothers Big Sisters, Military Affairs Committee, Great Falls Public School Advisory Council, Great Falls Development Authority and is privileged to serve as an Honorary Command Chief for the Global Strike Command Unit of the United States Air Force.

Introduction

Mark Cappis is the owner of Cappis Consulting and Tax, LLC, where he specializes in accounting, wealth management, and consulting. He has been involved in the Great Falls community through many organizations including the Great Falls Chamber of Commerce, Great Falls Development Authority, President of the Great Falls Kiwanis Club, Past President and Treasurer of the Great Falls Police Community Foundation, Boys & Girls Club, Past President and Treasurer of the Salvation Army Advisory Board, MSPCA, Great Falls Chapter of CPA’s, Past President and Treasurer of the Great Falls Children’s Receiving Home, Past President and Treasurer of the Lewis and Clark Foundation, and the Great Falls Fire Chief Search Committee. Mark also serves on the Statewide Board of the MSU Bobcat Board. Mark joined the Alluvion Health Foundation Board joined the board in the Spring of 2021. “Once I saw how much Alluvion was doing in the community and the people involved on the board I felt it was a great place to put some of my efforts into making my community a better place for everyone.”

Introduction

Kim grew up in a farming community in rural eastern Oregon and received her B.S in biology from Willamette University. It was there that she met her future husband, Bryan, and they returned to Great Falls where he was raised. Kim worked for several years at the McLaughlin Research Institute before taking a break to raise her three children. She has been active in the Great Falls Community for more than 20 years, serving on local PTA boards for many years where she enjoyed the opportunity to build relationships with teachers and students. It was during this time that she saw first-hand some of the hardships facing children and families in our community, and realized the schools were their lifeline. It was the schools who were trying to connect families to local resources and organizations for assistance. Kim also joined the Kids Education Yes committee and served with them for several years, spending countless hours campaigning for the passage of school levy’s. She also served on the Great Falls Symphony board for several years.

Kim and her husband own Jewell Properties, which holds nearly 80 residential rental units in Great Falls. She is very aware of the current housing crisis and the challenges being faced by our community. She is excited to learn about the different programming that Alluvion offers to help address the shortage of housing, and the services they provide for mental healthcare and addiction.

She and her husband Bryan are currently empty nester’s and like to spend their free time outdoors or traveling to visit their children.

ERICA HARP, BSN, RN, NCSN

Introduction

Erica Harp grew up in Great Falls and returned to her hometown in 2009. Erica has worked as an Emergency Department Nurse in Atlanta, GA, Spokane, WA and locally at Benefis and Great Glass Clinic. In 2011 Erica took a position with Great Falls Public Schools as the Lead School Nurse and developed a strong passion for school nursing. Erica raises her two children in Great Falls with her husband, Travis. She has been involved in the Great Falls community through organizations such as Young Life, Get Fit Great Falls, Action for Healthy Kids, Uptown Optimist, and serving her local church. Erica is a past President for the Montana Association of School Nurses and currently serves as a Director for the state of MT to the National Association of School Nurses.

Erica joined the Alluvion Health Foundation Board because of her long-held passion for community health. This passion started in nursing school and Erica is happy to be a part of serving her local community through the foundation. Erica has also been a partner in working with Alluvion Health through school-based health centers within Great Falls a Public Schools

Introduction

Apryle Pickering joined the board of the Alluvion Health Foundation in June 2022. As the Director of Clinical Care and Population Health at Blue Cross and Blue Shield of Montana she oversees healthcare management teams in utilization management, case management and wellness, as well as developing a comprehensive population health strategy for the MT plan. Apryle has over 20 years of healthcare experience leading medical groups in Western Montana. Apryle has led efforts supporting population health including home-based primary care, remote patient monitoring, care team development, and integrated behavioral health. Apryle received her Master’s in Medical Anthropology and her Master’s in Public Health from the University of Montana in Missoula and in 2018 was named one of Missoula’s Top 20 leaders under 40 for her work with population health and healthcare delivery transformation.
